ReConnected Care Plus Initiative
Reconnected is funded by the National Lottery Community Fund. We offer free support for people who are over 55 years of age for whom Covid-19 has exacerbated their isolation/loneliness and damaged their ability to engage with others. It’s completely free and available for residents of Stafford Borough and South Staffordshire.
Community Support Advisors can offer 1-1 support through:
– A telephone-based, response service to provide reassurance and reduce loneliness and feelings of isolation to those less mobile.
– Support for those that require more practical help and guidance,
for example through signposting and completion of referrals to specialist agencies.
– Assistance with accessing the relevant agencies to improve quality of li
fe and mental wellbeing. This includes helping to maximise income and benefit applications.
– Support to enable people to access engagement opportunities in their local community. This may include home visits to build confidence and give information on activities in the area, to assisting people unable to physically attend to access activities using oth
er methods to help them feel included in the local community.
In addition to the 1-1 support we provide, ReConnected also provide people-led activities to the over 55s in a variety of locations across Staffordshire, supporting improvement with both physical and mental health. The free activities and engagement opportunities are varied and offer people the opportunity to learn new skills, create healthier lifestyles and develop support networks.
